Once you sign up for a 15-day (or 100 signed waivers) free trial with a username and password, simply send your waiver as a Microsoft Word Doc or PDF (not a scanned image) to: support@smartwaiver.com. We are more than happy to convert your first waiver free of charge.
Note - Smartwaiver is able to extend this offer to users whose documents total ten pages or less and contain no more than 20 custom questions. Smartwaiver will not assist in creation/edit of the legal language found in the waiver.
You can also send us a waiver via the "Create/Edit Waivers" section of your account after logging into www.smartwaiver.com:
All smart waivers will be automatically formatted with a minimum of six required elements:
- Header/Title of the waiver
- Body of the waiver (provided by you)
- First & Last Name of the participant
- Date of Birth (for age verification) of the participant
- Electronic Signature Consent Checkbox
- Signature of the participant
You may add custom questions and fields you need to your waiver with the exception of: Social Security Numbers, Credit Card Numbers and Bank Account Information.
Please note: We are happy to provide free digitization services to our users who need to convert their documents into digital waivers. However, while most legal documents that require a signature can work with our software, we may be unable to extend our free digitization service to documents that fall outside the scope of what is considered a "waiver" such as: Intake Forms, Medical Assessment Forms and Physical Evaluation Forms.
