Once you've downloaded your CSV file, you may have some questions as to what all the data means.
Here's how to decipher each column:
First |
The first name of each participant who is covered by your waiver. |
Middle |
The middle name, if given, of each participant covered by your waiver. |
Last | The last name of each participant who is covered by your waiver. |
Date of Birth | The date of birth of each participant listed on the waiver is shown in mm/dd/yyyy format (month, day, year). A waiver containing multiple participants will show the date of birth pertaining to each specific participant. For example, a minor will show his or her own date of birth and not the parent or guardian's. |
Date Completed (UTC) | The date shown is the date the waiver was completed by your participant in yyyy/mm/dd format (year, month, date). The second group of numbers in this column is the time of day when the waiver was signed (for example: 20:05:41). We use a universal time called UTC. Click here for more information on UTC. |
Waiver ID | The waiver ID is a unique ID associated only with the waiver your participant signed. Sometimes you'll see the ID appear more than once on the CSV file. This occurs when there are multiple participants on the waiver, such as a parent and minors. This ID lets you know that your participants came from a shared waiver. |
Status | The status column shows the current state of the waiver at the time of downloading your CSV file. There are 3 possible states you will see: Pending Email Verification means that your participant has completed your digital waiver and received an email to verify that they are the owner of the email address they provided, but have not yet clicked on the verify link in their email inbox. Completed Online means that your participant signed your waiver and verified their email address online. Completed at Kiosk means that your participant signed your waiver at a kiosk location. |
Gender |
If you activated the gender question for your waiver then it will show the gender of each participant included on each waiver. The gender question is defaulted to off. |
Phone | Your participants phone number is shown in this column. It can be different for each participant included on the waiver. |
Custom Questions | The Waiver Editor allows you to create custom questions on your waiver which will show your participant's answers in the CSV file. Each column will be titled by the name of your custom question. |
Marketing Emails Allowed |
Underneath the space where your participant should provide his/her email, there is an optional checkbox which says Check to receive information, news, and discounts by email. If this box is checked then the CSV file column will be marked as Yes. |
Parent/Guardian First Name |
This column will show the first name of the parent or guardian on a waiver. Any minor who is included on a waiver will have the parent/guardian's name in this column. This column will be left blank if a waiver was signed by an adult with no minors included. |
Parent/Guardian Middle Name |
This column will show the middle name of the parent or guardian on a waiver if applicable. This column will be left blank if a waiver was signed by an adult with no minors included. |
Parent/Guardian Last Name |
This column will show the last name of the parent or guardian on a waiver. This column will be left blank if a waiver was signed by an adult with no minors included. |
Parent/Guardian Phone | This column holds the phone number specifically input by your participant for the parent/guardian. It will be left blank if only an adult is signing the waiver with no minors included. |
The email address entered by the participant will appear in this column. It will be blank if the waiver was filled out at a kiosk waiver which did not require an email address. | |
Title of Document |
You are able to create the title of the document in the Waiver Editor. If you go into the Waiver Editor and change the title of your waiver under the Header tab, you will see that the Title of Document column will show the new title name on the CSV file going forward with any new waivers completed. |
