Smartwaiver forms can accommodate up to one adult per form - or one adult and up to ten minors on a form together.
However, if your business/insurance/attorney/etc. requires you to only accommodate one person per form, you can adjust these settings accordingly to only allow one person per form.
To set the form for one Adult only (and no Minors):
If Minors are not allowed to participate in your activity or to fill out your waiver, you can remove them entirely from your form.
- Log into your Smartwaiver account and jump to your page of waivers here.
- Click the Edit icon (pencil icon) on the waiver you need to adjust.
- Within the waiver editing screen, click into the 'Minors' section > turn OFF the toggle that reads "Allow minors on the waiver" (shown in the screenshot below)
- When you are finished with all other changes, click the green 'Publish Waiver' button in the upper right corner to finalize all changes and publish.
Your form will now allow only adults to sign. (Smartwaiver accommodates one adult participant per waiver.)
To set the form for one Minor only (and no Adults):
If your form only applies to Minors (and adults will not be participating), you can remove the Adult option entirely from your form, and change the setting to allow only one Minor per form.
- Log into your Smartwaiver account and jump to your page of waivers here.
- Click the Edit icon (pencil icon) on the waiver you need to adjust.
- Within the waiver editing screen, click into the 'Minors' section > turn OFF the toggle that reads "Allow multiple minors on the same waiver" and the toggle that reads "Allow adults on the waiver" (shown in the screenshot below)
- When you are finished with all other changes, click the green 'Publish Waiver' button in the upper right corner to finalize all changes and publish.
Your form will now only allow minors to sign, and will now support just one minor participant per waiver.
